How to Request Your Health Care Records
Under Texas law, you have the right to request access to your health care records.
To request records from BTR Counseling, please follow the steps below:
Submit a written request by email or mail:

Mail: [Insert practice mailing address]Please include the following information in your request:
Your full legal name
Date of birth
Description of the records requested
Preferred format (electronic or paper, if applicable)
A copy of a valid photo ID
If requesting records for another person, documentation showing your authority to do so
Processing timeframe:
Requests will be processed in accordance with applicable Texas and federal law. You will be notified if additional information or fees are required before release.
